Carers
One in every eight adults is a Carer. A Carer is anyone who provides unpaid care for a family member or friend who is ill or disabled, or frail because of old age. If you are a Carer you can gain support from many different sources. The Carers Support Centre in Brigg offers free information and advice. If you are a Carer, please may you complete the form available below and return it to the surgery when possible. This ensures that the GP is aware that you are a Carer and can also offer support and guidance accordingly.

Consent to Disclose Medical Information
If a relative or Carer is required to have access to your confidential medical information, please complete the form below and send to Central Surgery. It is vitally important that this form is signed by you as the patient and by an independent witness who the consent is not being granted for. If for any reason it is felt the request is suspicious the practice with holds the right to ask for further information from the patient who the records belong to.
