You can register at our practice online or by calling in and asking at reception.
Click here to start the registration process online.
Please allow up to 7 working days for your registration to complete. Should you need to see a Clinician urgently please contact the surgery.
All patients will be registered with Central Surgery rather than a specific Doctor. Patients are entitled to express a preference and this will be recorded on their medical card when they register. This preference can either be given verbally to the Receptionist, or in writing on the medical card, or by letter. All patients may use either the Barton or Goxhill surgeries regardless of where in the practice area they live.
The practice does not discriminate on the grounds of: race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
Temporary Residents
If you are living within our catchment area temporarily and require medical advice you can register with Central Surgery on a temporary basis.
Please click here to download the form you need to complete and return to the surgery. Please see the above red text for how to return your form to the surgery. You cannot fill the form in online so you are required to print and complete manually.
Your Electronic Record and Information Sharing
Central Surgery uses a unique computer system called SystmOne that allows the sharing of full electronic records across different NHS healthcare services.
When you register with Central Surgery, clinicians at other organisations that care for you (e.g. out of hours and physiotherapy) and use SystmOne will be able to see the information recorded here. For example, a district nurse that visits you would be able to see the data entered by your GP. If you are not happy for your records to be shared, please let us know.
